This Privacy Policy describes how your personal and health-related information is collected, used, and protected by BlueVision Solutions (“we”, “us” or “our”) when you use the HEP Link mobile application (the “App”). Play or App Store require the app name and company name to be prominent in the first paragraph. HEP Link is a platform used by your Physical Therapy Provider (“Provider”) to prescribe and monitor your home exercise program.

1. Information HEP Link Collects

To provide a functional and effective recovery experience, the App collects the following information:

  • Account Information: Your email address and a password provided by your clinician to identify your specific exercise profile.
  • Health and Exercise Data: Information regarding your injury type, prescribed exercises, sets, reps, and frequency as prescribed by your Provider.
  • Compliance Data: Records of when you complete an exercise, your self-reported pain scores, difficulty ratings, and any comments you share with your Provider via HEP Link.

2. How Your Information is Used

The information collected is used solely for the following purposes:

  • Clinical Care: To allow your Provider to prescribe, monitor, and adjust your treatment plan.
  • Progress Tracking: To calculate “Total Points” and milestones to encourage your recovery within the App interface.
  • Communication: To facilitate feedback between you and your Provider regarding your exercise performance.

3. Data Sharing and Disclosure

  • Your Provider: Your data is shared directly with your Physical Therapist and their authorized clinical staff through a dedicated, secure database linked to HEP Link.
  • Third Parties: We do not sell, trade, or rent your personal or health information to third-party marketers.
  • Legal Compliance: We may disclose information if required by law or to comply with a judicial proceeding, court order, or legal process.

4. Data Security and HIPAA Compliance

We take the security of your Protected Health Information (PHI) seriously.

  • Encryption: Data is encrypted during transmission between your device and our secure servers.
  • Storage: Your data is stored in a HIPAA-compliant environment (Google Workspace) under a signed Business Associate Agreement (BAA) between BlueVision Solutions and Google.
  • Access Control: Only your authorized Provider and the App’s secure service account have access to your specific exercise database.

5. Your Rights

You have the right to:

  • Review the exercise and compliance data stored in your profile.
  • Request corrections to any inaccurate information.
  • Discuss the deletion of your account and data with your Physical Therapist.
